Participating Addendum

To participate, complete a Participating Addendum with the selected carrier(s). Send a copy of the completed Participating Addendum to the Contract Administrator.
 

When completing a Participating Addendum (PA) with carrier(s) the following information must be included:
Type of rates selected (e.g., single national carrier vs multiple national carrier or MBG vs Non-MBG) and the effective date of the PA.
